AGREEMENT TO PARTICIPATE IN A TRESILLIAN GROUP

I agree / understand that:

• The group builds on information from week-to-week and my attendance at each session is expected.
• The privacy of fellow participants must be maintained and personal information disclosed within a session must be held in confidence.

 

HEALTH INFORMATION AND PRIVACY

• I agree / understand that:• Health information will be treated in accordance with the NSW Health Records and Information Privacy Act 2002 and the NSW Health Privacy Manual.
• Documentation related to group registration and attendance will be recorded in the local health area’s electronic health record, including any clinical assessments undertaken.
• I am entitled to request access to my health information that is held by Tresillian.
• Data from clinical records will be used for administration purposes, internal quality assurance, auditing, service evaluation and research.
• Further details around health information and privacy are available in NSW Health’s Privacy Leaflet for Patients, a copy of which can be requested from Tresillian staff or accessed on NSW Health’s website.
• Tresillian staff are mandatory reporters and are required by law to report suspected child abuse and neglect to government authorities under NSW’s Children and Young Persons (Care and Protection) Act 1998.
